Athletic Training

Bellarmine athletics is focused on creating for its student’s opportunities that will become an integral part of the education and development of the "whole student".   Bellarmine Athletics is also striving for Championships and we are here to keep the Knights healthy and driving towards those goals.  It is our job to work with the student-athletes and to keep them on the playing field whenever possible. 

In order to help keep them playing and on the field we need to have a complete medical history.  This allows us to work with the student-athlete and our Team Doctors to care for them when necessary.  It is Bellarmine University’s and an NCAA policy that requires us to have a complete record of each athlete’s insurance coverage.  This policy requires that each athlete have Health and Accident Insurance in force while competing for Bellarmine University. 

Below you will find links to the forms that are required to be filled out and returned before any student-athlete is allowed to participate in Bellarmine Athletics.  There is also information for perspective student-athletes.  All of the forms listed below must be filled out and returned to The Head Athletic Trainer prior to August 1, every year.  Forms can be mailed to David Krebs, Athletic Trainer, Bellarmine University, 2005 Newburg Road, Louisville, Ky. 40205.
